add column headings to combat tracker

To make the tracker more intuitive for newcomers, it would be nice if each column had a heading. If these are abbreviated, you could use the tooltip to provide the full name and possibly some key notes on usage.

Looking closer at this, there are very few columns that have logical names - most are buttons to toggle views etc...

So not sure there is a lot to do here - other than maybe revisit the tooltips for the buttons to ensure they make some sense.

Documentation would always help too !
